本研究は、小海ら(2000 ,2004,2008)による日本語版 Mini-Mental State Examination(MMSE)の検出力と特異性を明らかにした。この MMSEの cut-off値を 24/25点とした場合、感度 0.837、特異度 0.957となり、臨床群( amnestic Mild Cognitive Impairment; MCIと probable Alzheimer's Disease; ADを含む)と健常群を判別するためのスクリーニングテストとして、十分な検出力と特異性を有することが示唆された。しかし、同様に 26/27点を cut-off値とした場合、感度 0.889、特異度 0.739となり、amnestic MCI群と健常群を判別するためのスクリーニングテストとして、十分な検出力と特異性を有するとは言い難く、他の認知機能検査による精査を行う必要性があることが示唆された。 |

In this study, we determine the sensitivity and specificity of a Japanese version of the Mini-Mental State Examination(MMSE)conceived by Koumi H(2000, 2004, 2008). When a cutoff score of 24/25 points was assumed for clinical patients(including patients with amnestic mild cognitive impairment(MCI)and probable Alzheimer's disease)and healthy elderly controls who had taken the MMSE, the sensitivity was found to be 0.837, and the specificity, 0.957. These sensitivity and specificity levels were considered to be sufficient for the MMSE to serve as a screening test to distinguish between clinical patients and healthy elderly controls. However, when the MMSE cutoff score for amnestic MCI patients and healthy elderly controls was assumed to be 26/27 points, the test was found to have a sensitivity of 0.889 and specificity of 0.739. These sensitivity and specificity levels were considered to be insufficient to support the use of MMSE as a screening test for distinguishing between amnestic MCI and healthy elderly controls. In this case, it was necessary to examine in detail the results of other cognitive functional tests in order to distinguish between amnestic MCI and healthy elderly controls. |
