| 内容記述 | 本研究は、イングランドの脱施設化の歴史と背景にある思想を辿るなかで日本にACTを普及するために必要なことを明らかにすることを目的にした。イングランドの精神科病床削減は、1954年に始まり、現在も引き続き進行中であり、急激ではないが、着実に減り続けている。一定の地域を定め、サービスがどの家庭にも届けられるよう配慮されている。特に北バーミンガムでの取り組みは、ケアマネジメントを機能分化させた上で、バーミンガム市の社会福祉局やボランタリーセクターと連携を図りつつ、国民保健サービス(National Healty Service:NHS)のいくつかの機能的チームが地域生活支援を展開している。包括型地域生活支援プログラム(Assertive Community Treatment:ACT)は、重い精神障害のある人への最も効果的な地域生活支援の方法である。英国の精神保健の歴史とバーミンガムの実践から、ACTを日本に普及するためには、1.援助者の人間観や援助観、2.ACTのための予算の確保、3.援助者の使命感、4.援助者の技能の向上、5.援助者の待遇改善の5点が必要であることがあきらかになった。 | |||||||||||

| 内容記述 | This present study is aimed at clarifying the necessary factors to promote ACT in Japan through reviewing the history of deinstitutionalization in England and its ideological background. The psychiatric hospital beds reduction in England started in 1954, and it is still in progress. The number of psychiatric hospital beds continues to decrease steadily in England. Attentions are paid to enable services to be provided to every family in a certain area. Especially, the Birmingham National Health Service system differentiates care management functions, and while the NHS cooperates with the Birmingham Social Services Department and many voluntary sectors, some multidisciplinary teams practice community living support for people with severe mental disabilities. Assertive Community Treatment is the most effective method of community living support. Mental health history in England and the practice in Birmingham show the following five points wiich could be helpful for promoting ACT in Japan: 1. Staff's philosophy about human being and helping; 2. The budget for ACT; 3. A sense of mission of the staff; 4. Enhancement of the staff's helping skills; and 5. Improvement of staff's working conditions | |||||||||||
